After School Membership Application 2020/2021
As part of the membership application, please print out, sign and have your child bring with them the first day they attend the club the following forms:
According to new COVID-19 policies and procedures, Club members must:
- Wear masks - if they don't bring one, we will provide one
- Attend Club 3 days per week to ensure spot
- Have a temperature of less than 100.3 to attend
- Complete a COVID-19 health questionnaire before entry
- Be picked up on time to keep their spot
